Our trained carers will provide medication support as documented in your personalised care plan
We assist with managing medication, including ordering and collecting prescriptions from the pharmacy and liaising with community healthcare services such as GPs, pharmacies, and district nurses.
Our trained and competent staff will provide medication support based on your personlised care plan to administer prescribed medication, whether from blister packs or original packaging.
Our trained carers will prompt you to take your medication or provide hands-on assistance in managing it
The scope of our support will be clearly described in both the service agreement and individual care plan we develop with you and/or your family or representative.
1. Prompting and Reminding: Our trained carers will remind you when it is time to take your medication.
2. Assisting: Our carers will help prepare medication (like opening blister packs or pouring liquids), provided you can take it yourself.
3. Administering: Our trained carers can physically give the medication. This includes tablets, inhalers, and applying creams.
